What Is NAD+ and How Do NAD+ Supplements Work?
NAD+ (nicotinamide adenine dinucleotide) is a coenzyme found in every cell. It helps transfer electrons during reactions that convert carbohydrates, fats, and proteins into ATP, the cell’s usable energy. It also supports enzymes involved in DNA maintenance, gene regulation, and cellular signaling. NAD+ is the oxidized form that accepts electrons, while NADH is the reduced form that carries them; the body continually converts one into the other.
The term “NAD+ supplement” can be misleading because products do not all contain the same active ingredient. Direct oral NAD+ and NADH products are available, but many supplements instead provide NAD+ precursors: compounds the body can use to make NAD+. Common examples include nicotinamide riboside (NR), nicotinamide mononucleotide (NMN), nicotinamide or niacinamide, and nicotinic acid. According to the NIH overview of niacin and NAD metabolism, absorbed forms of vitamin B3 can be converted into NAD, although their metabolic pathways and tolerability differ.
Products also vary by delivery method. Capsules, powders, sublingual liquids, and liposomal formulations are taken orally, while clinics may offer NAD+ injections or intravenous infusions. Claims that liposomal products, injections, or IV therapy provide superior benefits are not supported by equally strong clinical evidence. This distinction is essential when discussing NAD+ side effects. A reaction associated with high-dose nicotinic acid, for example, should not automatically be attributed to NR, NMN, NADH, or direct NAD+. Safety must be evaluated by the exact ingredient, dose, route, and duration of use.
